Disciplined for overtime claimed and approved by directors

I have been employed by company for 7 months, Last month whilst preparing the company overtime approval report to be signed off by the directors and all managers, I added my overtime to the report as I was not allowed to claim for the last six months but wanted to claim for this month and was expecting them to either approve it or deny it before approving the Overtime for payment. Both directors and all managers signed off on the report and I was not questioned as to why I have added my overtime so I submitted it to our finance department. 3 weeks later the Directors called me in for a meeting and asked me why I claimed overtime, I then explained to them why. A week later I was given a Notice of Disciplinary Hearing and the misconduct alleged is as follows. "Fails to comply with any provision contained in the policy of the employer resulting in financial loss for the employer". I understand that unlike the technicians working for us technicians I did not complete an overtime slip have it signed off, which is the standard protocol,  however the directors signed off the overtime report which I compiled which had all the supporting docs (timesheets), now they want to discipline me for financial loss. Is this right? Financial Loss? For overtime that was legitimate, overtime which they approved? Overtime which they admit that they are not questioning?
